I know this seems convoluted, but it is does happen and it causes Firefox to hang in such a way that only the Activity Monitor can close it (with FF3, it was a hard hang so you could force quit)

Put an extension XPI on your desktop.

Select File Open

Then drag the XPI onto the browser in the background.

The install dialog comes up but the file open dialog is on top and cancel no longer works on the file dialog.

You can't do anything with Firefox at this point.

You have to open up Activity Monitor and close it.

In FF 3, this was at least an app hang so you could force quit.

The reason this scenario can happen commonly is someone might go to open an XPI locally from their hard drive using file open and then drag and drop it to the browser instance instead of selecting open.
On Windows Vista I can't drag an xpi to the browser windows; I get a "Forbidden" cursor
