This happened to me twice:  On a thumbnail search result on images.google.com, I got an Unresponsive Script alert popup on the browser window while I had a Save Image As dialog open.  Pressing the Save or Cancel buttons did nothing, although other parts of the Save dialog continued to respond.  I couldn't click on the Stop Script or Continue buttons because the window wouldn't take focus back from the modal dialog.  The Quit command was disabled, too, so I had no choice but Force Quit.

When starting up again, it said it had trouble restoring tabs and gave me the list of windows and tabs; I unchecked the images.google.com one and it restored normally.
This isn't a bug with session restore. In fact, this is showing that session restore is working :)

I've had the same problem 4 or 5 times. I'm on Snow Leopard with all updates.

It occurs when I'm slow to select a location for the Save As dialogue (if it is left unattended for, say 30 sec).

No problems with restore, just a force-quit restart for Firefox.
