These additions would add functionality to the combined button and be more intuitive:
Open Address in New Tab
Fully Reload Current Page
‘Reload (override cache)’ is available via keyboard shortcuts but is not well known. It remains a valid troubleshooting technique, see here: http://en.wikipedia.org/wiki/Wikipedia:Bypass_your_cache (NB. Problems caused by a cache error remain even if the user attempts to load the same URL in a new tab.) ‘Fully Reload Current Page’ on the context menu makes the feature more user-friendly (by avoiding technical jargon) and discoverable. (It would also assist web designers, who are constantly updating page elements.)
‘Open Address in New Tab’ would add the useful, but again hidden, [Alt]+[Enter] feature to the combined button, making it more discoverable and accessible to more users. (A lot of people have trouble remembering or even executing keyboard combinations. Note also that on-screen keyboards for touchscreens may not include [Alt] keys.)
The grayed out ‘Move to Menu’ and ‘Remove from Toolbar’ would be completely hidden to remove clutter.

Sounds like a sensible idea.
I'd go so far to say that we can even get rid of the toolbars section for that specific context menu, since it doesn't really fit in there.
So the whole context menu for the stop/reload/go button would read as follows:
Open in New Tab
Fully Reload Page

Customize…

bug 1133001 points at https://twitter.com/benjaminhollway/status/561122925865345024, a screenshot of Google Chrome, where:
when Web inspector is opened, the Refresh button's context menu has:
* Normal Reload
* Hard Reload
* Empty Cache and Hard Reload.
Erwann proposes in that bug that something like that happen in Fx when devtools are opened.
