Before this patch, TYPE_REFRESH was not set by anything.

Olli, can I still land this? This only fixes a content policy type that wasn't being set by anything before.

I guess no, TYPE_DOCUMENT is used so much more often in the addons.

The proper fix for this will require a resolution to https://www.w3.org/Bugs/Public/show_bug.cgi?id=28361 so that we can know whether to use "metarefresh" inside frames and iframes...
