User Agent: Mozilla/5.0 (Windows NT 10.0; WOW64; rv:40.0) Gecko/20100101 Firefox/40.0 Build ID: 20150807085045 Steps to reproduce: Reloaded, reinstalled. No effect. Window controls do not change appearance with mouse-over. Min, Max, and Close do not function. Actual results: Completely inop.
OS: Unspecified → Windows 10
Hardware: Unspecified → x86_64

Are you in tablet mode? They work fine for me (and most other people) in normal windowed mode. (And I also get hover states in tablet mode.)

The problem was intermittent, but seems to be gone now. I apologize for the waste of time and thank you guys for your rapid responses. Love Firefox, and not going anywhere. Sorry again.

Did you find the root cause? Incompatible add-on? Theme issue? OS updates missing?
Still not working correctly in 40.0.2 Double clicking either of the 3 icons at top right of screen toggles the screen in and out of full screen. - and X not working at all.
Good news I am still on version 40.0.2 which still had the problem until I installed Stardock's Start10 Windows 10 alternative Start screen layout http://www.stardock.com/products/start10/ which cured the problem as well as improving the Start button functionality.
Not all the users using FF40 on Win 7 are affected. Is it a bug in Win 10? Is it triggered by a special theme or settings in Win 10?
I saw the same issue on SuMO and other places. I really think it may be caused by HiDPI + lwtheme https://support.mozilla.org/en-US/questions/1075202 In other words, I think screenshots would be helpful here.
Too late for 40 but we will take a patch for 41.
I have had this problem and it seems to be related to theme addons. I have used Littlefox for so long I forgot it wasn't part of FireFox. It works fine with FreFox 40** under Win 7, Win8.1, but NOT under Win 10. I suspect other theme addons may have the same problem.

Problem with extension Walnut 2 not showing min,max,close now disappeared. Working normally.
