Steps to reproduce:
BUG: add Firefox to the taskbar, launch Firefox => there are 2 icons on the taskbar :-(
Actual results:
there are 2 icons on the taskbar (see the attached screenshot)
Expected results:
Firefox should reuse the existant icon
After the update, metadata of the shortcut in the taskbar can change. Delete the previous shortcut and generate a new one to pin in the taskbar.

No, I don't have any previuos shortcut.

1. Install Firefox without creating icons(select custom, uncheck 'Create icons for Firefox' items)
2. Launch Firefox and pin it to taskbar

Try reinstalling Firefox with creating icons.(at least one of them) It won't harm your profile.

I was able to reproduce this issue on Windows 10 Pro x64 (Build 10525) using:
* Firefox 40.0.2 (20150812163655)
* Beta 41.0b3 (20150820142145)
* Aurora 42.0a2 (2015-08-23)
* Nightly 43.0a1 (2015-08-23)
with the steps provided by atlanto in Comment 4:
1. Launch Firefox installer.
2. Click "Next" → select "Custom" → click "Next".
3. Select destination folder then click "Next".
4. Uncheck the "On my Desktop" and "In my Start Menu Programs folder"
options and click "Next".
5. Leave "Launch Firefox now" option checked and click "Finish".
6. Right-click the Firefox application icon from the taskbar and
select "Pin this program to taskbar".
A second, separate Firefox icon is pinned onto the taskbar, displayed exactly as in the attached screenshot. Please note that this might be a Windows-related issue, as it's currently reproducible with other Windows desktop applications as well, e.g. Skype for desktop.

I upgraded to Windows 10 from Windows 7, and I have a firefox installation which recently upgraded to version 40.
It doesn't seem to be dependent on how exactly you start and pin Firefox. All of the following reproduce this issue:
method #1
• start firefox,
• right-click the icon in the taskbar
• choose "pin this program to taskbar";
method #2
• open the start menu
• right-click the firefox icon
• choose "pin to taskbar", then start firefox (in any way);
method #3
• go into the installation directory (eg. under Program Files)
• right-click the firefox.exe executable
• choose "pin to taskbar"
• then start firefox (in any way)

Apparently the taskbar ID, as can be found in HKEY_CURRENT_USER\SOFTWARE\Mozilla\Firefox\TaskBarIDs, is not added to the shortcut file "%AppData%\Microsoft\Internet Explorer\Quick Launch\User Pinned\TaskBar\Firefox.lnk" when pinning Firefox to the taskbar.
As a workaround, you can add the taskbar ID found in the registry key above to the .lnk file with https://code.google.com/p/win7appid/downloads/detail?name=Win7AppId1.1.exe
Log off and on and it should work.
