STR_1:
1. Open about:addons -> Plugins, set Flash to "Ask to activate"
2. Open attachment 8675621 [details]
3. Open about:addons -> Plugins, set Flash to "Never activate"
4. Switch to the tab from Step 2, click on the Flash plugin
5. Click "Allow now" in popup from identity block. 
6. Click "Allow..." in notification at the top of content area, click "Continue allowing" in popup.
7.(bonus) Repeat Step 6 several times

AR:  Step 6 can be repeated numerous times, which causes much confusion
ER:  Step 6 shouldn't be possible. Some obviously good behavior is expected

Example of good behavior: "After Step 5 browser should show the same UI it shows for disabled plugin".

Note:
If there're many tabs, it's too easy to browse a lot before encountering a tab with flash set to "Ask
to activate". If user disabled Flash long time ago, you can't expect him to remember that. Just like
you can't expect user to know that button "Allow now" will not enable Flash. UI should provide correct
information. I mean, isn't Mozilla expects user to be "average"? For example, options "Disable JS" and
"load images automatically" in preferences were removed for this exact reason: user isn't fully aware
of what he's doing. So expecting user to remember Flash state is rather strange after all that.
Even I can't do that, and apparently, I'm not average.
