In bug 1401392 I made a quick fix that results in each browser and content process serving its own application accessible to ATs.
As a follow-up, I think we should use mscom::PassThruProxy to send the browser's application accessible down to content so that we always serve the same object.
This also ensures that application accessible access is always in-process for injected DLLs.
