See bug 1436798, most of the commits shows an empty name in MozReview, and publishing review causes 500 internal server error.

looks like this was resolved by re-pushing the review.
i have no idea what happened originally - there's nothing in the logs that point to a failed transaction; it's especially puzzling because the review requests should't be able to be created without that data.
i'm going to mark this as resolved/worksforme only because it's working now (not because there wasn't a problem).
