It's possible the tab is discarded for inactivity, and then reloaded. Surely the favicons code doesn't issue any reload by itself. Please attach a log from your about:support page.

Ok, honestly you have a lot of enabled add-ons, that makes hard to understand if your problem is related to Firefox or caused by one those add-ons. I'd suggest to try reproducing the bug in Safe Mode (https://support.mozilla.org/kb/troubleshoot-firefox-issues-using-safe-mode) that will temporarily disable add-ons, and in a new profile (https://support.mozilla.org/kb/profile-manager-create-and-remove-firefox-profiles) to understand if it's add-ons or Firefox.

I need to be out in near future or some long time, and I test it briefly on 60 FDE, and managed to reproduce. And I believe you guys can also reproduce it if simply give a try. Especially when you have a a lot of tab and choose to "show you tabs and windows from the last time" on every restart.

those restored tabs are not really loaded, they are placeholders, and as such when you click on them they are effectively loaded and the favicon is loaded as well, I don't think there's a strict relation with comment 0.

Please try my STR in comment 0, you will find all favicons went blank.

This gif recording clearly shows favicon reloads itself. If I have had my network disabled, they will go all blank.
