Monitor breach indicators are inaccessible to screen reader users, as the indication is visual only (colour and background image). Even some sighted users may not understand what the icon is for and there's no way to find out without clicking on the account to view it, where the issue is described in text.
I would suggest the indicator be converted into a standalone element inside the login item, rather than a class on the item. We can then attach a title attribute to that element, which will serve as a tool tip for sighted users who don't understand what the indicator is for, as well as being included in the accessible name for the item.

I’ve reproduced this issue with Fx 70.0a1 (2019-08-23) on Windows 10 x64.
The issue is verified fixed with Fx 71.0a1 (2019-10-16) and Fx 70.0 across platforms (Windows 10 x64, macOS 10.14, Ubuntu 18.04 x64).
