Steps to reproduce:
Crazy bug that I think might have been introduced recently: when I fling scroll via touchpad (my laptop has a precision touchpad, not sure if that's relevant or not) , if I hit or hold "Ctrl" as the page is scrolling (fling scroll momentum) , it zooms out. Strangely though, when I fling scroll via touchscreen and hit "Ctrl", it doesn't zoom out.

Assuming you're on Windows based on your User Agent string, this is a known issue tracked in bug 1199737.
