We're getting some reports of idle timeouts with outstanding requests. That's probably under unusual conditions (30s is a long time), but it's probably worth doing something. Lower idle timeouts exist and we don't want requests to time out.
We'll need to build the infrastructure in neqo issue 1038 where we can work out the API and other things like timing.

I think we can do this in neqo and we do not need a change in necko.
