In testing, I have found that this bug is not 100% reproducible.
NOTE
In addition to occuring with new bookmarks (described in Step 1), this also
occurs when sequentially viewing different sites (with their icons already
defined) in your bookmark list.
STEPS
1. Start with a bookmarked SITE_1 that should have an icon but doesn't (because
you haven't been there yet; in my case this situation arose because I had copied
over my Mozilla bookmark file)
2. Visit the site through the bookmark: admire how its bookmark's icon is
updated to the site's icon
3. Visit a newly bookmarked SITE_2 that should have an icon but doesn't (a
bookmark just like in Step 1)
4. Open the bookmark menu and notice how both SITE_1 and SITE_2 now have the
icon of SITE_2

This is happening to me, too (with Phoenix 0.3 on WinNT) but I haven't been able
to create a testcase...
I clearly see the problem with the sites in my Bookmarks Toolbar, e.g. Yahoo,
Register.co.uk, Mozillazine, Novell, Slashdot, and Salon. All have site icons
but they don't always display them, and they sometimes get associated with other
sites that do *not* have site icons (clue?).
I can reproduce this. Add 2 bookmarks to the bookmarks toolbar, the first being
www.google.com, the second slashdot.org. Click on the link for Google, and then
after the page loads click on the link for Slashdot. The Google icon should
change to the icon for Slashdot. I tested this on 3 computers, all running
Windows 2000 SP3. It only seems to happen if the second site is Slashdot, but I
don't think it matters what the first one is.
