We should tracks HW decoder crashes in MediaFormatReader. We also should track crashes per audio/video decoder.

In addition, not related with this, another thing I'm thinking about is that in D142326 we would check the amount of crashes, the default value you set is 2, did you consider that value for only one decoder, or both decoder? Because one the RDD crash, it's possible that we would receive two crash error from each decoder.
