I'm also having this problem. My user agent: Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.5b) Gecko/20030903 Firebird/0.6.1+ Also, if a cookperm.txt file exists in the user's profile directory, entries from this file will be displayed in the Exceptions dialog. New entries cannot be added through this dialog, and existing entries cannot be manipulated in any way.

I can reproduce this with Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.6a) Gecko/20031020 Firebird/0.7+. -> Confirming as I can't find a related bug.

There have been a lot of work in the cookie backend lately. I don't know when this was fixed, but it now works for me using Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.6b) Gecko/20031114 Firebird/0.7+ (Steffen). Don't forget to click OK in the Cookie Manager _and_ in the options dialog.
