look here (sorry, its german):
http://forum.counter-strike.de/bb/thread.php?TID=72205
Reproducible: Didn't try
Steps to Reproduce:
1. visit a NEVER(!) ever visited website, which have no favicon (erasing the
cache ist not enough!)
2. bookmark this page
3. now upload a favicon.ico or favicon.png at the main folder of the webpage
4. visit the page again
5. now u can see the favicon in the titelbar, but it will be never saved to the
bookmark, u have to erase the boomark, and make a new bookmark to solve this*
*hint: look at your bookmarks.html and compare bookmarks with favicon and without
Actual Results:
Favicon was not added to the Bookmark
Expected Results:
Mozilla should save the "new" favicon automatically, or after a refresh "f5"
This bug is growing through the using of the "backup-export" funktion of the
bookmark manager, this "saves" that the page has no favicon, or have a favicon.
Thank you and good bye, sorry for bad english but -> running out of time :-)

Seems to be open season on favicons and "scripts on this page running
slowly".
I suspect that what you are hoping for could only be achieved by a system
in which every time the URL addressbar changes, an HTTP request is sent
for /favicon.ico (and possibly for any favicon(s) referenced in link elements).
How else could you know that the browser's idea of the current favicon
is the same as the server's and/or web author's?
I really hope that this is not implemented!
Other distributed databases such as DNS and caching proxies have
designed TTLs et cetera so that there is an agreed notion of freshness.
If this is important to you then I suggest that under the Tools menu
we have an item for 'Server Info' to show the latest known info about
the server for the current URL (or selected URL in a sidebar) including
its banner, canonical name, IP number, protocol and favicon.
In 'page info' we could add a tab for meta data including whether
its favicon came from a link element or /favicon.ico and so forth.
You could refresh this at will.
