It appears that Extensions.rdf is no longer created in
appdir/defaults/profile/extensions and that this file is copied over to new
profiles. Copying an Extensions.rdf from a zip build into
appdir/defaults/profile/extensions appears to resolves this issue and zip builds
are not affected since this file exists with them. This is fallout from bug 286034.

I'll do this on Monday unless Ben or someone gets to it sooner. I would like to
ditch the pre-installed extensions.rdf altogether.
