entered data required, ie bd, state, medicare effective date and buttons below were available but did not show image as to what they were for. Found out they should have said submit on the left button and cancel on the right button. Had to use Navigator to find out

This is fixed in trunk, and therefore most likely a dupe of that bug where styles were allowed to be applied to Cocoa widgets (which I can't find right now).
You should also e-mail AARP and tell them about this, because not having any text in the button is really bad for accessibility. For instance, a blind person's screen reader would have no idea how to differentiate between those buttons, so they really ought to have "Submit" and "Cancel" in the "value" property regardless of whatever CSS is applied to them.
I'm going to leave this UNCO for now so it can be duped properly.
