I'm sitting behind an Authenticating Proxy and I sometimes leave Firefox alive with many opened tabs while shutting down my system.
When restarting Firefox, it opens many proxy authentication dialogs, one for every tab, where only one can be used/closed.
If a wrong one is the topmost, you cannot close it and it seems firefox has crashed, because nothing happens wherever you click.
You have to move every topmost dialog away until you reach the (only) one that responds.
It seems that only one dialog at a time can be used. Maybe some better way to handle this would be to allow every dialog to be "used" in parallel, so the processing order should not be relevant.

Steps to Reproduce:
seems to happen stochastical, because I didn't find a way to reproduce it for shure...
1. configure some authenticating proxy
2. open many tabs
3. kill firefox
4. reopen firefox
=> many auth-proxy dialogs get spawned, sometimes the topmost is not the one that responds to interactions.
I use compiz as the window-manager. Maybe compiz changes the order in some circumstances which others do not...
