STR:
1. Create a fresh profile, or ensure that you have Minefield set to always check to see if it's the default browser on launch, and have another browser set as default.
2. Start Minefield.
3. Watch the "default browser" sheet fly in from the left side of the screen and attach itself to an approx 2px wide window, and watch all the application menus disappear from the menubar (except for the appname menu).
4. Dismiss the "default browser" sheet and watch it fly offscreen to the left.
5. Watch the first browser window load, and notice that there are still no other menus in the menubar. Notice that any keyboard commands that are defined in the missing menus fail.
6. Open Preferences, and notice menus are finally restored.
Ordinarily I'd suggest that the sheet was being attached to the hidden window instead of waiting for the browser window to appear, but I thought the hidden window had a full complement of menus to allow for normal menus when no (real) windows were open.
This regressed sometime between 3.0b4
Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.5; en-US; rv:1.9b4) Gecko/2008030317 Firefox/3.0b4
and the March 16 build I grabbed this week when I needed to look at something
Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.5; en-US; rv:1.9b5pre) Gecko/2008031604 Minefield/3.0b5pre
If you don't have the "default browser" check on (and are not starting a fresh profile, obviously), both of these behaviors go away. Not sure if there are other ways to get a sheet at startup that could possibly also trigger these bugs.
I suppose this might be a Cocoa Widget bug instead of a Firefox Startup one.
Attached you can see (mostly the drop shadow of) the 1-2px wide window (it's on top of the titlebar of the "untitled folder") and the floating sheet semi-attached to this crazy window, as well as the total lack of menus.

Yeah, I've definitely seen this and meant to file. Even made a video!

This is pretty horrible behavior and should probably block.
