If there is more than 1 tab open and another child window open (like the downloads window), clicking the close button in the main Firefox window (the x in the top-right-hand corner) only asks if you are sure you want to close Firefox as there are several tabs. However, if I select File>Exit, the correct "Save and Quit" option dialog appears.
Also, there is no dialog box of any kind (neither "Save and Quit" or "Close Tabs") if there is only 1 tab open. This means if you have navigated to a web page you want to return to, Firefox will not remember it unless you check the history log.
Reproducible: Always
Steps to Reproduce:
No save and quit
1.Open Firefox
2.Create at least 1 new tab
3.Select Tools>Downloads
4.Click the close button on the main Firefox window (x in the top-right-hand corner)
No dialog when closing
1.Open Firefox (ensure no other instances of Firefox are running or child-processes)
2.Go to any webpage (e.g news.bbc.co.uk) without creating a new tab or opening any other Firefox window.
3.Click the close button on the main Firefox window (x in the top-right-hand corner) or select File>Exit
Actual Results:
No save and quit
A dialog box appears saying "You are about to close X tabs. Are you sure you want to continue?" with Close Tabs and Cancel as the options.
No dialog when closing
Firefox closes instantly without any dialog box.
Expected Results:
No save and quit
A "Save and Quit" dialog box to appear, the same as when File>Exit is selected.
No dialog when closing
A "Save and Quit" dialog box to appear.

The first case you're describing is a duplicate of bug 417675. The second case is an intentional decision - we only show the quit dialog if more than one tab is open (or more than one window is open).
