tested with Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.5; en-US; rv:1.9.1b3pre) Gecko/20081210 Shiretoko/3.1b3pre
1. launch Shiretoko
2. browse any web page
3. File > Page Steup
4. in the Page Setup dialog, click on Cancel
5. the File > Page Setup menu entry is now permanently disabled...

Only if by "permanently" you mean "permanently until you focus another app and then go back to Fx."

I meant "permanently" because only crazy geeks like us are able to imagine
that focusing to another window and coming back will solve the issue. Most
people will shut down firefox to restart it, saying "this sucks".
