We went back and forth on this, but since we are adding "not now" over in bug 615318, we should make sure that the split menu for each notification contains the full set of commands. This is similar to the split menus in the Firefox button, where the main command appears again as the first item in the menu. The rationale is that users can continue to move forward through the interface as they make a choice, and never have to back track. Also, it is easier to consider the options (once, always, never, not now) when they are all shown inline next to each other. The downside of course it the added redundancy.
