Bug 400097 (bug 400097 comment 9) removed support for scheme: permissions (e.g., scheme:file for a pop-up or cookie whitelist/blacklist of file:// URLs). However, if a user had previously stored such a permission, it is not stripped during migration from hostperm.1 to permissions.sqlite. As a result, the user still has a permission visible in the UI that now does not work and has no idea why it's no longer working ;-) (See also bug 204285 and bug 670577 for people complaining that local-file permissions no longer work for things.) No idea what component the permissions manager lives in (does it have one?), but since bug 400097 was filed in Networking, this follow-up/regression will start out its life there, too.
