Starting with Thunderbird 9.0, after starting Thunderbird, the selected/shown message list columns are altered. The last time this happened the 'From' column had been deactivated ('From' seems to have been always affected). It can be reenabled and in the majority of startups survives in this configuration.
I have no steps to reproduce, but my main suspect is Test Pilot, because it has been added to Thunderbird in version 9.0 and I know its bugginess from Firefox.

Yes, still seeing with 10.0.2. If I remember correct, this happens often when the last selected folder (which will also get focus on startup) is a local folder containing only feed items.

This only happens with the first window loaded on startup, right-clicking a folder and choosing 'Open' opens a new folder and loads the correct columns and the 'Size' column which has also been added for the window loaded on startup.

This problem happens to me and my users too.
The problem seems to be random, some times passes a entire week starting and stopping TB everyday and the columns selected are respected, in other moments the configurations are lost everyday.
I can't to now figure out what changes.
Config:
Windows 7 SP1
TB 11.0.1

I haven't sure if I understood your question.
The problem happens without using safe mode (I never used it before)
But I tried this
* customized two columns
** Stared (put in the first column position of then grid)
** Account (put in the third position)
* restart with safe mode
** running in safe mode the columns stay on place
* stopped TB
* started TB normally
** Stared column disappeared
** Account column moved to he last grid position

I did some researches and at least 1 of my users complains about losing columns configurations very soon after TB versions upgrades.
Can be my case too, I haven't way to have sure.

I am unsure about this bug being a duplicate of bug 710056. They have in common that the column 'Size' gets added. I wasn't able to reproduce this problem at my last two restarts (maybe a [Test Pilot] update fixed it).
