Details
Firefox got updated to 14.0.1 last night. Since then, the location bar does suggests URLs from history again despite being configured to only suggest from bookmarks.
(might be related to #659437)

Bug 740076 disabled autoFill when "Nothing" was selected, but apparently didn't address the case where only "Bookmarks" was selected.

you can disable autofill by flipping browser.urlbar.autoFill preference

Correct, distinguishing history from bookmarks is sort of complicated. maybe we could just disable autoFill when the pref is set to only bookmarks.
