Steps to reproduce: 1. Load Create Collection page https://addons-dev.allizom.org/en-US/firefox/collections/add 2. Complete the mandatory fields and add a theme to your collection (ex. NASA Galaxy ) 3. Create the collection. Expected results: The collection is successfully created. Actual results: Oops page is displayed. Notes/Issues: Verified on FF20 (Win 7).
