Steps to reproduce: 1. Load Create a New Theme page https://addons-dev.allizom.org/en-US/developers/theme/submit 2. Enter a theme name, an already used slug (ex. money), category, header and footer images, agreement checkbox and click Submit Theme 3. Click the Submit Theme button again Expected results: There are no design or layout issues. Actual results: The header and footer images are no longer displayed. However, if the user enters a valid slug and press the Submit Theme button again, the theme is submitted successfully. Notes/Issues: Verified on FF20 (Win 7).
