Looks like it is always faling during the "Mini-manifest app name is different from webapp manifest name" step. We're using a custom 30 seconds timeout, is it possible that it is not enough time?

Could be, the B2G emulator is notorious slow. One of reasons timeouts in tests are explicitly discouraged :)

Looks like the removal of the custom timeout fixes the intermittent failure. It should also fix the latest intermittent failure in bug 915879.
