This bug covers restyling the generic and Firefox OS feedback forms and related flow pages (thank you, unknown product, ...). All of these template, css and js files should be in fjord/feedback/ .

This blocks the tracker bug #968414.

Thoughts: * John Slater suggested that we start with the Sandstone theme and work from there. * The generic form is currently being used for both desktop and mobile devices. Having a responsive form layout would be good. * The Firefox OS form should work on both desktop and mobile, too. * It'd be great if we didn't change any strings. If we have to change strings, then so be it. This includes adding new error messages. I'd like the next step to be screenshots so we can run those by Matt and Cheng. We want to make sure the new style doesn't affect our ability to get new feedback and thus they need to ok the designs.

Couple of notes about the above screenshot (attachment 8392486 [details]): - The layout adapts to fit the screen width and gives the largest possible click targets. - Click targets are made clearer by the addition of the white box - Colors are made to match the regular input design. - Subtle font color and size differences change the focus. - There is better padding for better readability.
