Details
Shift + [Reload Button] does a forced refresh of a website, telling caches and
proxies to ignore what they think they know and check the source for sure. This
options isn't available through the menus, and should be a seperate menu item
alongside Reload to show users there are multiple options.

Ah, if this were a native Mac app, we'd provide this feature by making the
`Reload' menu item become `Super Reload' when the Option key was held down.
That's what 3.x did. But since we can't (and IMO shouldn't be able to) do that
in XUL, I really don't know we could provide access to this feature in a
non-confusing manner.
