The Preferences window needs more options to manage images to be blocked.
You should be able to add new as well as edit blocked images.
Reproducible: Always
Steps to Reproduce:
1. Click on Edit - Preferences
2. Select Privacy & Security and then Images
3. Click View Image Permissions
Actual Results: Only has Remove buttons
Expected Results: Needs add and edit buttons
The reasoning for this is say I only want to block images in a certain section
of a site or even just block a single image, then I can just add this to the
list in the preferences or edit a current on.

Possible way to edit the current Image Manager (based on bug 109810 mentioned
above).

Basically the idea behind all of this is that it would block all images from
www.mozilla.org except for images in the /gif/ folder and subdirectories or the
specific cool.gif.
By allowing so called EXCEPTIONS a user can customize and chose what they need
to block like in order to block certain ads but allow regular site images
through.
SUGGESTED RULES:
1. Sites should be listed by domain (and alphabetically second) IE,
images.mozilla.org should be right before www.mozilla.org and not after
www.google.com. This way a user can keep easy track of exceptions to the rule
for example they have images blocked on part of the site and allowed on other
parts.

I think this bug can be closed down as RESOLVED -> WONTFIX
Looking back, the kind of features I was looking for are now served by adblock's fine-grained control.
It might be nice if Firefox built in such fine-grained control, but that's really a big topic that should be dealt with separate from my rather simplistic suggestion I offered here. I would like to close this report for no longer being relevant, but just wanted to look for any feedback before I did. :)

If you really would like it reopened, feel free to do so. However, personally, I think if Firefox really wanted to add something like this, it should be something more powerful like adblock's ability to handle both whitelists, blacklists, page elements, and regular expressions.
