When I add a bookmark to the bookmark list the according shortcut icon
(favicon.ico) is visible in the list or in the personal toolbar.
Next time I start Mozilla the icons are gone. But not for all the bookmarks and
not everytime. It is somewhat random.

1. Add a bookmark with a shortcut icon
