- about:preferences search textbox (and other textboxes in this page, like in Manage Permissions under Cookies and Site Data)
- about:addons search textbox
- about:networking#dnslookuptool and about:networking#logging
- about:telemetry search textbox
- about:url-classifier
- about:devtools Email textbox
There may be more.

I'm not sure what inconsistency you mean. The focus ring is intentional, see bug 1502684 and bug 1512332.

Yeah, I know.
By "inconsistency" I refer to the inner border's thickness in a textbox's focus ring, e.g. in the first screenshot the left and top inner borders are thicker than the right and bottom ones.
In the second screenshot the top inner border is thicker than the rest (except maybe for the right border, can't really discern).
See the in following screenshot, the "Keyboard shortcuts" inner borders have all the same thickness.
Hope this description is better.

Are you using a DPI setting greater than 1 and lower than 2?

Bug 1516767 could have regressed this too.
Will check for regression range later today when I get the chance (if Alice won't do it until then :-) ).
