I see this in Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.7)
Gecko/20040620, but not in the build from 20040619.
If I use Customize Toolbar to remove the "Location" field from the toolbar, the
Menu Bar and the "Customize..." command become grayed out for the rest of the
session.
After a restart of Firefox, the Menu Bar and the "Customize..." command are
activated again, but the Bookmark Toolbar Items are gone from the toolbar. Using
"Customize..." to add the "Location" field to the toolbar, will again deactivate
the Menu Bar and the "Customize..." command, but after closing and restarting
Firefox again, all is well.

I can reproduce this in 2004-09-07-07-0.9.
Mozilla/5.0 (Windows; U; Win 9x 4.90; ja-JP; rv:1.7.3) Gecko/20040907
Firefox/1.0 PR (NOT FINAL)
I got error message in JavaScript console:
