gtk theme: clearlooks, from svn
works for me using clearlooks or human on Ubuntu- Mozilla/5.0 (X11; U; Linux i686; en-US; rv:1.9b2pre) Gecko/2007120104 Minefield/3.0b2pre ID:2007120104
man, you're using old clearlooks and older human.
here you go, highlighted ui bugs related to this icon.
Not sure if this is a theme bug or a gtk widget bug.

It was probably Firefox bug. Now I see well displayed icon on all themes.

Huh... Tested increased size to 22x22 and icon is scaled down to fit sidebar's title...
It never was WORKSFORME, because noone tested it as should.
